Shoes and Tights Order Form
Please fill out the order form to let us know if your dancer needs shoes or tights. Please complete a form for each dancer if you need to order shoes for multiple dancers. Please see the tights sizing chart below, as the staff does not size dancers for tights.
Payment:
All orders will be placed on your account. Once received, we will either send your dancer home with in-stock items or order accordingly. If you would like to pay via check, please write it to Premier Dance Academy and give it to your instructor, or you may pay online via credit card.
Pricing:
All Tights: $12
White Leather Ballet Shoes: $22 plus tax
Patent Leather Tap Shoes: $28 plus tax
Jazz Shoes: $32 plus tax
Shoes by Age Group

Preschool dancers wear white leather ballet shoes.

Ages 3-4, dancers wear white leather ballet shoes and patent leather tap shoes.

Ages 4-5, dancers wear white leather ballet shoes, black Tap Shoes, and jazz shoes. Dancers may dance barefoot for class.

Ages 5-7, dancers wear white leather ballet shoes, black Tap Shoes, and jazz shoes. Dancers may dance barefoot for class.

Ages 8-10, dancers wear white leather ballet shoes, black Tap Shoes, and jazz shoes. Dancers may dance barefoot for class.

Ages 9-12, dancers wear white ballet shoes, black Tap Shoes, and jazz shoes. Dancers may dance barefoot for class.
All male dancers will need black ballet shoes and black jazz shoes. All female dancers will need tan jazz shoes.
